Follow Up

Follow up is where outcomes and revenue are protected.

Every important recommendation should end with a clear answer to six questions.

The Six Questions

  • What is the next step?
  • Who owns it?
  • When will it happen?
  • What barrier may stop it?
  • What support is needed?
  • How will we confirm completion?

A Strong Follow Up Script

Script

“Before we end today, let us make sure the next step is clear. What are you going to do, when are you going to do it, and what could get in the way?”

Staff Should Document

  • The recommendation discussed.
  • The client response.
  • The barrier identified.
  • The support offered.
  • The resource provided.
  • The follow up plan.
  • Any refusal.
  • Any supervisor consultation needed.

If There Is No Follow Up

  • If there is no follow up, the recommendation can disappear.
  • If the recommendation disappears, the client may disengage.
  • If the client disengages, services can stall.
  • If services stall, documentation, outcomes, and revenue can be affected.

Follow Up Is Part of the Work

Follow up is not extra. Follow up is part of the work.
